The Community Service Award honors an individual or organization that has made significant contributions to the community and to citizens of the Commonwealth to reduce heart disease and stroke.The Media Partnership Award honors an individual or organization that has made significant contributions to increasing awareness of heart disease and/or stroke, risk factors, or warning signs through the media.The Public Policy Award honors an individual or organization that has supported or championed issues that impact heart disease and/or stroke through policy and environmental change at the local and/or state level.
The Clinical Improvement Award honors an individual or organization that has made significant contributions to the quality of healthcare services in the Commonwealth to reduce heart disease and stroke.
